SERVICES & PROGRAMS
Gemstone offers long-term skilled nursing and end-of-life care which are supported by a range of specialty programs and services. Together with our compassionate, personalized approach, these capabilities have helped establish our long-standing reputation for excellence in senior care.
24-HOUR SKILLED NURSING CARE
Gemstone provides 24-hour care and support services for elderly residents and people with a chronic condition or a prolonged illness that requires constant monitoring. Our staff is devoted to the creation of a comforting respectful environment, and we are focused on helping each person achieve the highest possible quality of life.
Gemstone’s skilled nursing team provides:
- Long-term care, residential care and complex care services
- Medication administration services
- Individualized care plans
- Assistance with activities of daily living
- Incontinence, catheter & colostomy care
- Therapeutic and special diets
- Diabetic management
- Post-hospital and post-surgical care
24-hour nursing care is provided daily by professional Nurses and Resident Care Aides (RCAs). The provincial target for hours of service is 2.80 hours per resident day of direct care with a minimum of 20% of the total time performed by nurses. It is important to know that the targeted hours are managed on a facility-wide basis since some neighbourhoods may require additional resources during temporary times of transition.
ALZEIMER'S AND DEMENTIA CARE
We respect all our residents as individuals, taking the time to get to know them so we can personalize the way we care for them. Our approach is “resident first- dementia second” and our care revolves around each person’s own needs, not fixed rules and routines.
The consistent, stable staffing at Gemstone creates a warm and caring home-like environment for those afflicted with Alzheimer's disease or other forms of dementia. Specially trained to care for people with these diseases, our staff creates individualized care programs that build on each resident's strengths and abilities.
Your loved one will have the ability to participate in individual and group activities with other residents, ensuring that each day at Gemstone is filled with moments your loved one will enjoy. The security of our Residential Neighborhoods offer you peace of mind while the familiar surroundings of the center and the many activities make every day fulfilling for your loved one.
Our residents are important to us, and we enjoy building relationships with the people closest to them. Family and loved ones often wish to be part of care home life – from joining the Resident & Family Council, to organizing activities and helping out in the garden. Relatives often say this helps them to feel closer to their loved one, and that they’re playing a key part in their care.
RECREATION & THERAPY SERVICES
At Gemstone, we believe that the drive to stay involved, to grow and to enrich your life does not need to end as we age and in fact, we can still thrive well into our late years. At Gemstone, we encourage our residents to continue with a favorite activity, explore something new, and find out what helps nurture your mind, body and spirit. Our Recreation Programs are designed to give you choices to keep living life to the fullest and our Recreation Therapist ensures the content and variety of offerings provide the best value.
The monthly calendar is carefully planned to ensure each resident has an opportunity to participate in a variety of meaningful activities and we regularly monitor participation and enjoyment as a means to continuously change and improve the offerings.
We also offer an Occupational Therapist and/or Physiotherapist are available on a part-time basis for residents needs. They work to assess residents and make recommendations regarding proper seating and equipment fitting, exercise regimes and rehabilitation programs. All residents are assessed and wheelchairs (or other equipment) ordered before they leave the hospital. In addition, we work closely with our suppliers to help select specialized equipment for resident needs.
Our Social Worker supports the care team by providing admission & discharge planning, counseling & advocacy as well as education. This is a part-time role and the service is offered at times that align with resident needs. As an advocate for the vulnerable older adult, the social worker has a unique perspective in understanding systems within Brocklehurst Gemstone, the community and within the family. The social worker helps navigate complex systems to ensure services are identified and accessed. They also provide information relating to adult abuse and crisis intervention, as needed.
Dining is meant to be a pleasurable experience which is why the goal of our dining services is to provide enjoyable, delicious meals that of course, meet nutritional needs. We believe that dining excellence supports good health and helps promote quality of life.
The Food Services Manager at Gemstone oversees the dining services department and is responsible for providing 3 meals and 2 snacks daily for regular, texture-modified and therapeutic diets. Two choices of entrée and dessert are offered at lunch and dinner. The menu is modified twice yearly in the Spring/Summer and Fall/Winter with input from residents and their family. Village Council (Resident Food Committee) meetings are held at Gemstone to address menu and dietary concerns, make suggestions for specific changes to the menu and to provide input for resident choice meals and theme meals. To ensure quality nutritional care, Gemstone employs a Registered Dietitian to assess and monitor residents at nutritional risk.
Residents and families are invited to submit favorite recipes for menu ideas. We also welcome you to buy a guest meal ticket and dine in with us.
